Professional autonomy of nurses as an integral part of nursing practice
Abstrakt:
Východiská: Profesijná autonómia sestier predstavuje významný predpoklad samostatného rozhodovania, uplatňovania odborných kompetencií a efektívneho výkonu ošetrovateľskej praxe.
Cieľ: Analyzovať mieru profesijnej autonómie sestier v jednotlivých dimenziách ošetrovateľskej praxe.
Súbor a metodika: Prieskumu sa zúčastnilo 143 sestier. Údaje boli zbierané prostredníctvom skrátenej verzie štandardizovanej škály Dempster Practice Behaviors Scale (DPBS) obsahujúcej 28 položiek v štyroch dimenziách. Zber dát prebiehal formou online dotazníka v období február - marec 2025. Údaje boli analyzované pomocou deskriptívnej štatistiky.
Výsledky: Celkové priemerné skóre škály DPBS dosiahlo hodnotu x̄ = 1,79 (SD = 0,36), čo indikuje priaznivú mieru profesijnej autonómie. Vyššie hodnoty boli zaznamenané v dimenziách ocenenie (x̄ = 1,31) a pripravenosť (x̄ = 1,58), nižšie v dimenziách splnomocnenie (x̄ = 2,00) a aktualizácia (x̄ = 2,08), pričom vyššie hodnoty sa viazali na osobnú zodpovednosť a sebadôveru a nižšie na organizačné aspekty výkonu praxe.
Záver: Uplatňovanie profesijnej autonómie sestier je významne podmienené pracovným prostredím. Systematická podpora samostatného rozhodovania, jasné vymedzenie kompetencií a redukcia administratívnych bariér môžu prispieť k posilňovaniu autonómneho výkonu ošetrovateľskej praxe.

Abstract:
Background: Professional autonomy of nurses represents an important prerequisite for independent decision-making, the application of professional competencies, and the effective delivery of nursing practice.
Objective: To analyse the level of professional autonomy of nurses across individual dimensions of nursing practice.
Methodology: The study sample consisted of 143 nurses. Data were collected using a shortened version of the standardized Dempster Practice Behaviors Scale (DPBS), comprising 28 items across four dimensions. Data collection was conducted through an online questionnaire between February and March 2025. The data were analysed using descriptive statistics.
Results: The overall mean score of the DPBS reached x̄ = 1.79 (SD = 0,36), indicating a favourable level of professional autonomy. Lower mean scores (reflecting higher autonomy) were observed in the dimensions of appreciation (x̄ = 1.31) and readiness (x̄ = 1.58), while higher scores (indicating lower autonomy) were found in empowerment (x̄ = 2.00) and actualization (x̄ = 2.08). Higher autonomy was mainly associated with personal responsibility and professional self-confidence, whereas lower autonomy related primarily to organisational aspects of practice.
Conclusion: The implementation of professional autonomy among nurses is significantly influenced by the work environment. Systematic support for independent decision-making, clear definition of competencies, and reduction of administrative barriers may contribute to strengthening autonomous nursing practice.
